- Secure Patient Login - patient can login to the portal from a computer outside the enterprise
- Registration, Health History, PRO Completion - patients can complete their CCR or specific Registration and Health History Forms, as selected and defined by the hospital or practice
- Patient Reported Outcomes - QoL assessments in their secure portal. Assessments can bedelivered at specified time points (i.e. pre-treatment, 3 month, 6 month, 12 month, etc).
- Reminders & Messaging - triggers set by the provider will send email/SMS text alerts and reminders to the patient and medical team
Through its APIs and Partner Program, VTOC can integrate with any commercial EHR or Practice Management vendor. The VTOC integrated Medical Team & Patient Portal system enables:
- Pharmacy, Lab Results, Procedure Eligibility - data access
- Treatment Summary and Relevant Education Materials - meeting ARRA CCD/CCR data exchange requirements
- Reminders, messages and Forms - triggered based on data values
- Data Analysis and Trend Reporting - configurable for specific quality measures and patient-level reporting
- Data Registry Reporting - VTOC aggregates and exports the required patient-level data to the registries required by the HITECH Act
- Clinical Trials - patient-level data collection, protocol management and clinical trial matching & recruitment
